Application Directions
NYS daycare regulations require Discovery Kids to gather certain information about each child at the time of initial application and annually thereafter (September). Five required forms can be downloaded, completed, and emailed or mailed to the address below.
Application Documents
All documents must be completed to process your application along with a $75 initial enrollment deposit and reapplication fee.
The final document OCFS – 4433 is a form from the NYS Office for Children and Families’ that can be found here and downloaded. This form must be completed by your child’s pediatrician’s office, and returned to us before your child can start the program. Please note that many pediatricians have created their own form that mirrors the information on OCFS – 4433. That is acceptable as well.
Health Forms
Health forms are not required for your application unless indicated by Discovery Kids staff. If you are required to submit health forms, please download, fill out, and submit them to Discovery Kids.
Additional Forms
If you wish to add a day to your child’s schedule please complete this form and submit to the Director at least 48 hours in advance.
